We offer individualized personal care assistance to people with intellectual and developmental disabilities in their homes and in the community. We don’t provide programs but rather support individuals and their families in creating self-determined lives.
Advocating for a Good Life
The SDS group provides a support network for families who are responsible for individualized personal care assistance to people with intellectual and developmental disabilities.
This occurs in their homes and in the community utilizing Self-Directed Support services and other DMH/DHSS agency programs.
✔ Objective 1
Existing or potential families who utilize SDS services will be effective advocates by gaining and sharing institutional knowledge and strategies.
✔ Objective 2
Group participants that use SDS will have access to available resources and information to help them achieve their goals to care for their family member(s).
✔ Objective 3
SDS group participants provide advocacy for the group by meeting with the state-appointed fiscal agent, state level SDS or DMH administrators periodically as needed to effectively utilize SDS services.
✔ Objective 4
SDS group participants provide advocacy to members by meeting with policymakers, DMH/DHSS agency leadership, other government professionals, and community leaders.
✔ Objective 5
The SDS group will hold regular meetings on topics of interest to families using or considering SDS services and provide networking opportunities.
✔ Objective 6
The SDS group will hold a bi-annual conference on topics of interest to families and professionals supporting families that address best practices, current trends for residing in the natural home, and instructional sessions to meet required procedures, documentation, and ISP’s.
